Job Description

An opportunity is available for an experienced and compassionate Registered Nurse with a background in home health. Youll play a crucial role in supporting patients as they recover, manage conditions, and strive for wellness in their own environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

What Youll Do:

  • Conduct comprehensive patient assessments using OASIS and develop individualized care plans.
  • Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic disease support.
  • Monitor patient status, recognize changes in condition, and coordinate closely with other healthcare providers.
  • Educate patients and caregivers regarding treatment, medications, and preventive health.
  • Ensure timely, accurate documentation using Home Care Home Base (HCHB).
  • Work collaboratively with the interdisciplinary teamtherapists, social workers, and aides.
  • Participate in case conferences, staff meetings, and ongoing professional education.
  • Travel daily within your assigned territory with mileage reimbursement.

Desired Background:

  • Active RN license (in good standing).
  • At least 1 year recent home health experience (fieldwork preferred).
  • OASIS and HCHB proficiency required.
  • Strong assessment, organizational, and independent decision-making skills.
  • Valid drivers license, reliable transportation, and insurance.
  • BSN a plus.
  • Experience handling 2532 points/week and knowledge of Medicare/home health compliance are preferred.

$2,490 - $2,634/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
