Deliver compassionate, high-impact care as a Hospice Registered Nurse supporting patients and families during pivotal moments. This full-time contract opportunity focuses on providing holistic end-of-life nursing in home or residential settings, allowing you to make a meaningful difference every day.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional weekend or on-call rotation
• Field-based role with approximately 45 patient visits daily

Key Responsibilities:

• Conduct comprehensive assessments and develop individualized hospice care plans
• Deliver direct skilled nursing care emphasizing pain control, symptom management, and patient comfort
• Administer medications and treatments per hospice protocols and physician orders
• Continually evaluate and report changes in patient status to the interdisciplinary team and hospice medical director
• Guide, educate, and support patients, families, and caregivers throughout the hospice journey
• Collaborate with social workers, aides, chaplains, and volunteers to meet all dimensions of patient and family needs
• Ensure timely, accurate documentation in Home Care Home Base (HCHB)
• Participate in team meetings and quality initiatives to sustain excellence in hospice care
• Comply with all hospice regulatory standards and organizational policies

Qualifications:

• Active RN license in good standing
• At least 1 year of recent hospice nursing experience
• Proficiency with Home Care Home Base (HCHB)
• Strong skills in clinical assessment, symptom management, and family communication
• Reliable vehicle, valid drivers license, and auto insurance
• Ability to independently navigate a field-based schedule
• Compassion, empathy, and emotional strength
• BSN and palliative experience preferred
• Familiarity with Medicare hospice regulations is a plus

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
