An opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care in a home health setting. This role is ideal for a clinically skilled RN who thrives working autonomously, is organized, and passionate about providing patient-centered care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete OASIS assessments and create individualized, goal-driven care plans for each patient.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Continuously monitor patient conditions, update care plans, and communicate changes promptly with the healthcare team and physicians.
• Educate patients and families on managing diagnoses, medications, and preventive health strategies.
• Document all care accurately and efficiently within Home Care Home Base (HCHB) system.
• Coordinate patient care in collaboration with physical and occupational therapists, social workers, and home health aides.
• Attend case conferences, team meetings, and participate in ongoing education as needed.
• Uphold compliance with all regulatory and clinical documentation requirements.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least one year of recent field-based home health experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment and decision-making skills.
• Valid drivers license, reliable transportation, and current auto insurance.
• Ability to self-manage schedule and work independently across patient homes.
• Exceptional organizational, time management, and communication skills.
• Preferred: BSN degree and experience managing a caseload of 2532 points weekly; understanding of Medicare and home health standards.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
