An opportunity is available for a skilled and compassionate RN with Home Health experience to deliver patient-centered care in a field-based setting. This role is ideal for a self-motivated professional comfortable providing independent, high-quality nursing services to diverse patients in their homes.

Schedule and Assignment Details:

• Contract: Full-time, 13-week assignment
• Hours: Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (35 patient visits per day)
• Setting: Field-based, providing care in patient homes
• Technology: Utilizes Home Care Home Base (HCHB) for documentation; OASIS tool for assessments
• Travel: Daily visits within an assigned territory; mileage reimbursement provided

In this capacity, you will:

• Complete comprehensive OASIS assessments and design individualized care plans
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management
• Closely monitor patient progress, promptly reporting changes to healthcare teams and physicians
• Educate patients and families on disease processes, care regimens, and medication administration
• Maintain accurate and timely documentation in HCHB
• Work collaboratively with an interdisciplinary team to ensure the highest standard of coordinated patient care
• Participate in ongoing education, team meetings, and case conferences
• Ensure adherence to all clinical, regulatory, and documentation standards

Preferred background includes:

• Active RN license (in good standing)
• At least 1 year of recent Home Health nursing (field experience strongly preferred)
• Proficiency with HCHB and OASIS assessments required
• Strong clinical, critical thinking, and communication skills
• BSN and prior management of a points-based caseload preferred
• Reliable transportation, valid drivers license, and proof of insurance

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
