An exciting contract opportunity is available for a dedicated Home Health Registered Nurse (RN) to deliver high-quality patient care in a home setting. This role centers around fostering positive patient outcomes and collaborating with multidisciplinary teams while maintaining clinical excellence and compassionate communication.
Schedule:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM 5:00 PM
- Field-based with daily travel to patient homes
- Caseload: 2532 points per week (typically 35 visits daily)
Key Responsibilities:
- Complete thorough OASIS assessments and design individualized care plans.
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, swiftly recognizing changes in status and communicating with healthcare providers as needed.
- Educate patients and families on diagnoses, home treatment, medication regimens, and preventive care.
- Document clinical encounters accurately and promptly using Home Care Home Base (HCHB).
- Foster collaboration with physical therapists, occupational therapists, medical social workers, and home health aides.
- Participate in interdisciplinary meetings, case conferences, and ongoing training as required.
- Comply with all regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing in the state of practice
- At least 1 year of recent home health field experience
- Proficiency in Home Care Home Base (HCHB) and OASIS
- Strong assessment, clinical, and organizational skills
- Valid drivers license, reliable transportation, and proof of auto insurance
- Ability to work independently, manage time effectively, and uphold professional standards
- BSN and prior experience managing 2532 points per week preferred
- Familiarity with Medicare and home health compliance is a plus
