Job Description
Experienced and compassionate nurses are needed for an engaging role focusing on home health patient care. As a Home Health Registered Nurse, youll provide vital skilled nursing services to patients in their homes, helping to ensure continuity and a patient-centric approach. This contract position offers the opportunity to manage your own caseload while collaborating closely with an interdisciplinary care team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
In this full-time, 13-week contract, youll be expected to complete 2532 points per weekusually three to five patient visits each day. With daily travel required, mileage reimbursement is available. Electronic documentation is completed using Home Care Home Base (HCHB), including comprehensive OASIS assessments.
Primary Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress and communicate changes promptly with care teams and physicians
- Educate patients and their families about treatment plans, disease management, and medications
- Maintain accurate, up-to-date documentation in HCHB
- Coordinate with therapists, medical social workers, and home health aides
- Attend case conferences, staff meetings, and continue professional development
- Ensure compliance with regulatory and agency standards
Preferred Experience:
- Active RN license (in good standing) in the state of practice
- At least one year of recent home health experience (field experience strongly preferred)
- Proficiency in HCHB and OASIS
- Reliable transportation and valid drivers license
- Outstanding clinical assessment, critical thinking, and organizational skills
- BSN and prior experience managing full home health caseloads preferred
- Familiarity with Medicare and home health compliance
