Job Description

An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health care. This contract position offers the chance to make a meaningful impact through delivering patient-centered care directly to individuals in their homes. The role requires a self-motivated professional who excels in independent environments, maintains clinical rigor, and communicates effectively with patients and interdisciplinary teams.

Schedule & Assignment Details

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 2532 points per week (typically 35 patient visits per day)
  • Field-based: Daily travel within an assigned territory (mileage reimbursement included)

Key Responsibilities

  • Perform comprehensive OASIS assessments and establish individualized care plans.
  • Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease interventions.
  • Monitor patient conditions, recognizing and reporting any changes to the broader healthcare team and physicians.
  • Provide education to patients and families about diagnoses, therapies, medications, and preventive measures.
  • Complete timely and accurate documentation using Home Care Home Base (HCHB).
  • Maintain ongoing collaboration with physical therapists, occupational therapists, medical social workers, and home health aides.
  • Participate in case conferences, team meetings, and ongoing professional development.
  • Ensure compliance with regulatory and agency standards for patient documentation and care.

Qualifications

  • Active RN license (in good standing) in the relevant state.
  • Minimum 1 year of recent Home Health nursing experience (field experience preferred).
  • Proficiency with Home Care Home Base (HCHB) and required OASIS assessment experience.
  • Strong assessment and critical thinking skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to manage a flexible, mobile schedule and communicate effectively.
  • BSN and experience with Medicare compliance are preferred.
