Make a meaningful impact in patient care as an experienced Home Health Registered Nurse, supporting individuals in their homes with high-quality, compassionate nursing. This field-based contract position offers both professional independence and the chance to be an integral part of a collaborative, multidisciplinary care team for a 13-week engagement.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for patients.
• Deliver skilled nursing services including wound care, IV therapy, medication administration, and chronic disease management.
• Evaluate patient condition, monitor for changes, and maintain detailed communication with physicians and care teams.
• Educate patients and families regarding care plans, medications, and disease management to encourage optimal outcomes.
• Complete accurate, timely documentation utilizing Home Care Home Base (HCHB).
• Cooperate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure cohesive care.
• Participate in case conferences, staff meetings, and ongoing education as required.
• Uphold regulatory and agency standards in all aspects of patient care and documentation.
• Daily travel within the assigned territory, with mileage reimbursement provided.

Qualifications:

• Active RN License in good standing (state of practice).
• Minimum of one year recent Home Health experience, preferably with field-based work.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment and problem-solving skills.
• Valid drivers license, reliable transportation, and current auto insurance.
• Effective communication, time management, and organizational abilities.
• BSN and experience managing a 2532 point caseload preferred.
• Familiarity with Medicare and home health compliance guidelines is beneficial.
