Experienced and compassionate registered nurses passionate about delivering patient-centered care in the home setting will thrive in this field-based position. As a Home Health RN, you'll play a critical role in supporting individuals with acute or chronic health needs through comprehensive assessments, skilled interventions, and effective care coordination.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract assignment
• Caseload: 2532 points per week (approximately 35 patient visits daily)

Key Responsibilities:

• Complete OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing services such as medication administration, wound care, IV therapy, and chronic disease management.
• Monitor patient status, communicate changes promptly with the care team, and ensure timely physician updates.
• Educate patients and caregivers about diagnoses, medications, treatments, and preventive measures.
• Maintain accurate, up-to-date clinical documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary healthcare professionals to ensure seamless, well-coordinated care.
• Participate in case conferences, staff meetings, and professional development.
• Travel daily within the assigned territory; mileage reimbursement included.

Required Qualifications:

• Active RN license (in good standing) in the state of practice
• Minimum of 1 year recent home health (field) RN experience
• Proficiency with HCHB and OASIS
• Strong clinical assessment, organizational, and communication skills
• Reliable transportation with valid drivers license and insurance
• Ability to work independently and manage a flexible, field-based schedule

Preferred:

• BSN
• Previous experience managing a 2532 point caseload per week
• Knowledge of Medicare regulations and home health standards
