Make an impact as an experienced and compassionate Home Health Registered Nurse, providing high-quality care to patients within the comfort of their own homes. Your expertise will ensure that patients receive personalized clinical attention while fostering positive health outcomes and meaningful patient relationships.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (generally 35 patient visits per day)
• Field-based role with daily travel to patient homes (mileage reimbursement provided)
• Full-time, 13-week contract

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services such as wound care, medication management, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and coordinate with healthcare team members and physicians.
• Educate patients and families on treatment plans, medications, disease processes, and preventive strategies.
• Maintain timely and accurate clinical documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care delivery.
• Engage in case conferences, staff meetings, and ongoing continuing education.

Qualifications:

• Active RN License (in good standing) in the state of practice.
• At least 1 year of recent Home Health experience (field experience highly valued).
• Experience using Home Care Home Base (HCHB) and OASIS assessment tool is required.
• Strong assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently within a flexible field-based environment.
• BSN preferred; knowledge of Medicare and home health compliance standards is an advantage.
