Deliver expert nursing care in a rewarding home health setting, where your skills and compassion will directly impact patients' lives. We are seeking a dedicated Registered Nurse (RN) with strong home health experience to join a patient-focused team providing care in a field-based environment.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Manage a caseload of 2532 points weekly (typically 35 patient visits per day)
• Daily travel within assigned territory (mileage reimbursement provided)

Responsibilities:

• Complete comprehensive assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing services including wound care, medication management, IV therapy, and chronic disease education.
• Monitor patient status, identify changes in condition, and communicate developments to physicians and the interdisciplinary team.
• Provide thorough patient and family education to promote health management and safety.
• Ensure timely, accurate documentation with Home Care Home Base (HCHB).
• Collaborate with physical and occupational therapists, home health aides, and medical social workers to support optimal patient outcomes.
• Attend case conferences, staff meetings, and essential ongoing training.
• Maintain compliance with regulatory and agency standards.

Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent field-based Home Health RN experience
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Exceptional clinical assessment, organizational, and communication abilities
• Valid drivers license, reliable transportation, and auto insurance
• BSN and experience managing 2532 points per week preferred
• Understanding of Medicare and home health regulations a plus

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
