An opportunity is available for an experienced and compassionate Registered Nurse specializing in home health care. This role is essential for delivering skilled nursing services directly to patients in their homes, ensuring a high standard of patient-centered care and clinical excellence. The position requires strong autonomy, organizational skills, and a commitment to supporting patient well-being in a field-based environment.

Schedule and Role Details:

• Full-time, contract position (13-week duration)
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Field-based, daily travel to patient homes within an assigned territory

Key Responsibilities:

• Complete comprehensive OASIS assessments and create individualized plans of care
• Deliver skilled nursing services including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate promptly with the healthcare team
• Educate patients and families on disease processes, medications, and preventive care
• Document care accurately and efficiently using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members to ensure integrated, high-quality care
• Participate in case conferences and adhere to regulatory documentation standards

Qualifications:

• Active RN license (state of practice) in good standing
• Minimum one year of recent home health experience (field experience preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and auto insurance
• Strong assessment and critical thinking abilities
• Excellent communication, time management, and organizational skills
• BSN preferred; experience managing 2532 points per week desirable; awareness of Medicare and home health compliance standards beneficial
