We're seeking a skilled and compassionate Registered Nurse with expertise in home health to deliver high-quality care to individuals within their community. This contract position offers autonomy, responsibility, and the chance to collaborate closely with other healthcare professionals to ensure exceptional, patient-centered outcomes.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Continuously monitor and evaluate patient status, communicating changes to physicians and other care providers.
• Educate patients and family members on disease management, treatment protocols, and prevention strategies.
• Complete timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate effectively with therapists, social workers, and home health aides for cohesive patient care.
• Participate in case conferences, staff meetings, and continuing education.
• Maintain compliance with agency and regulatory documentation and care standards.

Requirements:

• Active RN license, in good standing.
• At least one year of recent home health experience (field preferred).
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible field schedule.
• Reliable transportation, valid drivers license, and auto insurance.

Preferred:

• BSN
• Experience managing a caseload of 2532 points per week
• Familiarity with Medicare and home health compliance
