Make an immediate impact in patient care by providing skilled nursing services to individuals in their homes. We are looking for an experienced, compassionate Registered Nurse with home health expertise to deliver high-quality, patient-focused care and coordinate with an interdisciplinary team.

Schedule:

• Full-time, 13-week assignment
• Monday Friday, 8:00 AM 5:00 PM
• Daily home visits within assigned territory (mileage reimbursement provided)
• Caseload expectation: 2532 points per week (typically 35 visits per day)

Key Responsibilities:

• Perform comprehensive assessments using OASIS and develop individualized patient care plans.
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease monitoring.
• Monitor and document patient progress using Home Care Home Base (HCHB); communicate changes promptly to the wider care team and providers.
• Educate patients and families regarding conditions, treatments, medications, and preventative measures.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for comprehensive care.
• Participate in case conferences, staff meetings, and professional training.
• Adhere strictly to regulatory standards and organizational protocols.

Required Qualifications:

• Active RN license (in good standing)
• At least one year of recent home health RN experience (field-based preferred)
• Hands-on experience with OASIS and Home Care Home Base (HCHB)
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, time management, and communication skills
• Ability to work independently and manage a field-based schedule

Preferred:

• BSN
• Experience managing 2532 points per week
• Knowledge of Medicare and home health regulations
