Job Description

A skilled and compassionate Home Health Registered Nurse is sought to deliver high-quality nursing care directly to patients in their homes. This is a full-time, 13-week contract role designed for a motivated RN who thrives working independently and is committed to clinical excellence and outstanding patient communication.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans tailored to each patients needs.
  • Deliver skilled nursing services, such as wound care, medication management, IV therapy, and chronic disease interventions.
  • Track patient status, respond proactively to changes, and collaborate with physicians and the interdisciplinary care team.
  • Provide education to patients and families, enhancing understanding of conditions, treatments, medications, and preventive strategies.
  • Ensure meticulous electronic documentation in Home Care Home Base (HCHB).
  • Work closely with physical therapists, occupational therapists, medical social workers, and home health aides for seamless care coordination.
  • Participate in meetings, case conferences, and mandatory training or continuing education.
  • Uphold rigorous standards for documentation and regulatory compliance.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • At least one year of recent home health RN experience.
  • Proficient with Home Care Home Base (HCHB) and OASIS assessments.
  • Demonstrated strength in clinical assessment, independent decision-making, and effective communication.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • BSN, knowledge of Medicare/home health compliance, and prior experience managing high caseloads are preferred.
