Job Description

Providing skilled nursing care in patient homes, this opportunity calls for a motivated and organized Home Health Registered Nurse (RN) to deliver high-quality, patient-centered care. The role requires independent field-based work while ensuring clinical excellence and clear communication with patients and healthcare teams.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and care teams.
  • Educate patients and families on health conditions, treatment plans, medications, and preventive measures.
  • Maintain precise, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, medical social workers, and home health aides.
  • Participate in case conferences, meetings, and continuing education.
  • Comply with regulatory standards and agency policies for documentation and care.

Qualifications and Desired Experience:

  • Active RN license in good standing.
  • Minimum of 1 year recent home health nursing experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS mandatory.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Effective communication, time management, and organizational skills.
  • BSN preferred.
  • Experience managing a caseload of 2532 points weekly.
  • Knowledge of Medicare and home health compliance guidelines.

Job Details:

  • Location: Waltham, MA
  • Contract Type: Full-time, 13-week contract
  • Schedule: Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week; typically 35 patient visits daily
  • Travel: Daily travel to patient homes within assigned territory with mileage reimbursement
