Hospice Registered Nurse (RN) Walterboro, SC
Full-time | 13-week Contract
A compassionate Registered Nurse with hospice experience is needed to provide high-quality end-of-life care to patients in home and residential settings. This role involves managing patient comfort and dignity while working closely with a multi-disciplinary team to meet physical, emotional, and spiritual needs. The position is field-based and requires reliable travel within the assigned service area.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation may be required
Responsibilities:
- Perform comprehensive nursing assessments and develop tailored hospice care plans
- Deliver skilled nursing care focused on pain control, symptom management, and patient comfort
- Administer medications and treatments per physician orders and hospice protocols
- Monitor patient condition changes and communicate promptly with the hospice medical director and team
- Educate patients, families, and caregivers on disease progression and care strategies
- Collaborate with social workers, chaplains, aides, and volunteers to address holistic patient needs
- Maintain accurate, timely documentation using Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and quality improvement initiatives
- Ensure compliance with hospice policies and regulatory requirements
Qualifications:
- Active RN license in good standing for the state of South Carolina
- Minimum 1 year recent hospice nursing experience
- Proficient in Home Care Home Base (HCHB) documentation system
- Strong clinical assessment and communication skills
- Knowledge of pain management and family support in end-of-life care
- Reliable transportation, valid drivers license, and insurance for travel to patient homes
- Ability to work independently in a field-based environment
- Compassionate with emotional resilience
Preferred:
- BSN degree
- Experience in palliative or end-of-life care
- Familiarity with Medicare hospice documentation and regulations
