An experienced Home Health Registered Nurse (RN) is needed for a full-time, 13-week contract position in Virginia Beach, VA. This role involves providing skilled nursing care in patient homes, delivering high-quality, patient-centered care while working independently in the field.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate with healthcare teams and physicians.
• Educate patients and families about disease management, treatments, medications, and preventive care.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure coordinated patient care.
• Participate in case conferences, staff meetings, and continuing education.
• Comply with all regulatory and agency standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum 1 year recent home health experience, preferably field-based.
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
• Strong clinical assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with a flexible schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance standards.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM.
• Typically 35 patient visits per day with a caseload of 2532 points weekly.
• Daily travel within assigned territory with mileage reimbursement.
