A fulfilling opportunity awaits an experienced and compassionate Registered Nurse specializing in Home Health settings. As a Home Health RN, you will deliver high-quality nursing care directly in patients homes, ensuring continuity and a patient-centered approach throughout the course of care. This contract position offers the chance to make a profound difference in the well-being of individuals recovering or managing health conditions in their home environment.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent Home Health experience
- Field experience strongly preferred
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- OASIS assessment tool experience
- Reliable transportation, valid drivers license, and current auto insurance
- Strong assessment, organizational, and communication skills
- Ability to work independently and efficiently manage a field-based caseload
- BSN and knowledge of Medicare/home health compliance standards preferred
Responsibilities:
- Conduct comprehensive OASIS assessments and craft individualized care plans
- Deliver skilled nursing including wound care, IV therapy, medication administration, and chronic disease management
- Monitor and document patient progress, promptly communicating changes to the care team and physicians
- Educate patients and families regarding treatment plans, medications, and preventive strategies
- Maintain timely and accurate records in Home Care Home Base (HCHB)
- Collaborate with an interdisciplinary team for coordinated patient care
- Participate actively in case conferences, staff meetings, and ongoing professional development
- Consistently adhere to all regulatory and clinical documentation standards
