Put your clinical expertise to work as a Home Health Registered Nurse (RN) providing skilled care to patients in their own homes. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing activities such as wound care, medication administration, IV therapy, and management of chronic diseases
- Monitor patient progress and promptly address changes in condition, coordinating with the care team and physicians
- Educate patients and families on health conditions, treatment regimens, medications, and preventive measures
- Accurately document all care and communication in Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides to provide coordinated, holistic care
- Participate in case conferences, team meetings, and professional development activities
- Ensure adherence to all regulatory and organizational standards
Requirements:
- Active RN license in good standing for the state of practice
- Minimum of 1 year of recent home health (field) experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment and critical thinking skills
- Valid drivers license, reliable transportation, and auto insurance
- Independent work style and effective time management
- BSN preferred
- Familiarity with Medicare and home health compliance standards beneficial
