An opportunity is available for an experienced and compassionate Registered Nurse to provide skilled care in a home health setting. This contract position involves working independently in the field, delivering high-quality, patient-focused nursing services to individuals in their homes. The ideal candidate will demonstrate a strong sense of organization, effective communication, and dedication to achieving positive patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Daily travel required within the assigned territory

Key Responsibilities:

• Perform comprehensive OASIS assessments and create individualized care plans
• Administer skilled nursing care, including wound care, IV therapy, and medication administration
• Monitor patient progress and identify changes in condition, promptly communicating updates to care teams and physicians
• Educate patients and families on care plans, medications, disease management, and preventive care measures
• Accurately maintain documentation within the Home Care Home Base (HCHB) system
• Coordinate care with interdisciplinary teams, including therapists and home health aides
• Participate in case conferences, staff meetings, and ongoing education
• Ensure compliance with all regulatory standards and care protocols

Qualifications:

• Active RN license in the state of practice
• At least 1 year of recent Home Health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS
• Strong clinical assessment, critical thinking, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently in a flexible, field-based environment

Preferred:

• BSN
• Experience managing a 2532 point weekly caseload
• Familiarity with Medicare and home health regulations

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
