Job Description
An outstanding opportunity is available for an experienced and compassionate Registered Nurse with a background in home health. This field-based contract role centers around delivering skilled nursing care directly to patients in the comfort of their homes, ensuring a high standard of clinical quality, continuity, and patient-focused care.
Schedule and Assignment Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Expectation of managing a weekly caseload of 2532 points, typically 35 patient visits per day
- Field-based: travel to patient homes within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop care plans individualized to each patient.
- Administer skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize condition changes, and communicate promptly with other care team members and physicians.
- Educate patients and families about disease processes, treatments, medications, and preventative care.
- Document all care and communication thoroughly and accurately using Home Care Home Base (HCHB).
- Coordinate with therapists, medical social workers, and home health aides to ensure holistic patient support.
- Participate in interdisciplinary conferences and continue personal clinical education as necessary.
- Follow regulations and agency protocols for documentation and care delivery.
Required Qualifications:
- Active RN license in good standing for the state of practice
- At least 1 year of recent home health experience (field experience preferred)
- Strong proficiency with Home Care Home Base (HCHB) and OASIS
- Excellent clinical assessment, communication, and organizational skills
- Reliable transportation, valid drivers license, current auto insurance
- Ability to self-manage a dynamic, field-based schedule
Preferred:
- BSN
- Familiarity with Medicare/home health compliance
- Experience with managing a caseload of 2532 points per week
