Job Description

An opportunity is available for an experienced and compassionate Registered Nurse specializing in home health care. This role focuses on providing high-quality clinical care to patients within their homeswith a strong emphasis on autonomy, patient-centered service, and interdisciplinary collaboration. The position is a full-time, 13-week contract designed for individuals who excel at independent field-based work while maintaining clinical excellence and clear communication with both patients and care teams.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Role Highlights & Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized plans of care.
  • Deliver skilled nursing services such as wound management, medication administration, IV therapies, and chronic disease education.
  • Monitor patient conditions, recognize variations in health status, and communicate effectively with physicians and the care team.
  • Provide education to patients and families regarding care plans, medications, and preventive practices.
  • Document all care activities promptly and thoroughly using Home Care Home Base (HCHB).
  • Coordinate with interdisciplinary professionalsincluding physical and occupational therapists, medical social workers, and home health aidesto ensure seamless patient care.
  • Participate in team case conferences, professional development meetings, and adhere to documentation standards and regulatory guidelines.
  • Manage a caseload of 2532 points per week (typically 35 visits daily) with reliable daily travel to patient homes (mileage reimbursement provided).

Desired Qualifications:

  • Active RN license in good standing in the state of practice.
  • Minimum of one year of recent home health (field-based) nursing experience.
  • Proficiency with HCHB and OASIS assessments.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Strong assessment, critical thinking, communication, and organizational skills.
  • BSN and experience meeting Medicare compliance are preferred.
