Job Description
Deliver compassionate, high-quality care directly to patients in their homes as an experienced Home Health Registered Nurse (RN). This contract position places you at the forefront of patient-centered nursing, providing skilled clinical services and education to individuals who benefit from support in their home environment. You'll work independently while collaborating with a dedicated interdisciplinary team to drive positive health outcomes and enhance quality of life.
Schedule/Hours:
- Full-time, 13-week contract
- MondayFriday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based with daily local travel (mileage reimbursement provided)
Key Responsibilities:
- Complete comprehensive OASIS assessments, developing individualized care plans.
- Deliver skilled nursing: wound care, IV therapy, medication administration, and chronic condition management.
- Monitor patient progress; promptly identify and communicate health changes to the care team.
- Provide education to patients and families regarding treatment, medications, and ongoing self-care.
- Accurately document all care within Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and aides to ensure seamless care coordination.
- Participate in case conferences, team meetings, and continuing education.
- Maintain compliance with all regulatory and agency standards.
Qualifications:
- Current, active RN license in the state of practice (in good standing)
- Minimum one year of recent home health nursing experience (field-based preferred)
- Experience with Home Care Home Base (HCHB) and OASIS required
- Demonstrated clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Strong communication, time management, and organization skills
- BSN and experience managing 2532 points/week preferred
- Knowledge of Medicare/home health compliance a plus
