An exciting opportunity awaits experienced and compassionate Home Health Registered Nurses with a passion for delivering high-quality patient care in a home-based environment. This full-time, 13-week contract role provides the chance to work autonomously while making a significant impact on patients lives by providing skilled nursing care, education, and support.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload expectation: 2532 points per week (typically 35 patient visits per day)

Responsibilities:

• Conduct comprehensive OASIS assessments to develop and manage individualized care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, promptly identify changes in condition, and communicate with the healthcare team and physicians.
• Educate patients and families on care plans, medication regimens, disease processes, and preventive strategies.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with multidisciplinary team members, including therapists, social workers, and home health aides, to ensure seamless care.
• Participate in case conferences, staff meetings, and professional development activities.
• Adhere to all agency and regulatory standards for patient care and documentation.
• Travel daily within the assigned territory; mileage reimbursement provided.

Qualifications:

• Active RN license in good standing (state of KY).
• Minimum of one year of recent home health nursing experience (field experience highly preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, communication, and organization skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.
• BSN and experience managing 2532 points per week preferred.
• Knowledge of Medicare and home health compliance standards desirable.
