Deliver high-quality, patient-centered care as a Home Health Registered Nurse during a dynamic 13-week contract. This field-based nursing opportunity is key in improving patient outcomes and supporting individuals to receive exceptional clinical care in the comfort of their own homes.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Responsibilities:

• Complete comprehensive OASIS assessments and formulate individualized care plans.
• Provide direct nursing care, including wound management, medication administration, IV therapy, and chronic disease oversight.
• Monitor and evaluate patient progress, quickly addressing clinical status changes with physicians and care teams.
• Educate patients and their families about disease processes, care plans, medication regimes, and preventive measures.
• Maintain meticulous documentation using Home Care Home Base (HCHB).
• Work collaboratively with an interdisciplinary team (therapists, social workers, aides) to facilitate holistic care.
• Participate in staff meetings, interdisciplinary case conferences, and professional development activities.
• Ensure adherence to state, federal, and agency-specific documentation and care requirements.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent home health nursing experience (field experience highly preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation is required.
• Demonstrated clinical judgment, assessment, and critical thinking abilities.
• Valid drivers license, reliable transportation, and required insurance.
• Ability to independently prioritize and manage a flexible field-based caseload (2532 points weekly, typically 35 daily visits).
• Strong communication and organizational skills; BSN and prior management of high visit volumes preferred.
• Knowledge of Medicare and home health compliance is a plus.

$1,908 - $2,019/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
