An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes. This role focuses on providing quality, patient-centered care while working independently in the community. Strong clinical skills, organizational ability, and effective patient communication are essential for success in this position.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans.
• Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detect changes in condition, and communicate promptly with healthcare teams and physicians.
• Educate patients and families on treatment plans, medications, and preventive care practices.
• Complete accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary professionals such as physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with all regulatory and agency standards regarding patient care and documentation.

Qualifications and Desired Experience:

• Active RN license in good standing within the state of practice.
• Minimum of 1 year recent home health experience, preferably with fieldwork.
• Proficiency in using Home Care Home Base (HCHB) software.
• Experience with OASIS assessments.
• Strong clinical assessment and critical thinking capabilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible, field-based schedule.
• Excellent communication, organizational, and time management skills.
• BSN preferred.
• Experience managing caseloads of 2532 points per week.
• Knowledge of Medicare and home health compliance standards is a plus.

Schedule and Location:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM 5:00 PM.
• Field-based visits to patients homes within a designated territory.
• Caseload expectation: 2532 points weekly (35 patient visits per day).
