Job Description

A skilled and compassionate professional is needed to deliver high-quality nursing care to patients in their own homes. This 13-week, full-time contract position offers the opportunity to make a meaningful impact, while enjoying a supportive environment and structured schedule.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based, covering daily patient visits (35 visits per day; 2532 point caseload per week)

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans tailored to patient needs
  • Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient progress, promptly communicate changes to physicians and the healthcare team
  • Educate patients and families on care management, medications, and preventive health practices
  • Maintain thorough and timely documentation in Home Care Home Base (HCHB)
  • Coordinate care with therapists, home health aides, and medical social workers
  • Participate in team meetings, case conferences, and ongoing professional development
  • Uphold agency, state, and federal regulations for quality and documentation

Required Qualifications:

  • Active RN license (in good standing) in the state of practice
  • Minimum of 1 year recent home health field experience
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments
  • Reliable transportation, valid drivers license, and auto insurance
  • Effective communication, clinical assessment, and organizational skills
  • Ability to work independently in the field

Preferred Qualifications:

  • BSN degree
  • Experience managing 2532 point caseloads per week
  • Strong knowledge of Medicare and home health compliance standards
