Job Description
A skilled and compassionate professional is needed to deliver high-quality nursing care to patients in their own homes. This 13-week, full-time contract position offers the opportunity to make a meaningful impact, while enjoying a supportive environment and structured schedule.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, covering daily patient visits (35 visits per day; 2532 point caseload per week)
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans tailored to patient needs
- Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, promptly communicate changes to physicians and the healthcare team
- Educate patients and families on care management, medications, and preventive health practices
- Maintain thorough and timely documentation in Home Care Home Base (HCHB)
- Coordinate care with therapists, home health aides, and medical social workers
- Participate in team meetings, case conferences, and ongoing professional development
- Uphold agency, state, and federal regulations for quality and documentation
Required Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum of 1 year recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Reliable transportation, valid drivers license, and auto insurance
- Effective communication, clinical assessment, and organizational skills
- Ability to work independently in the field
Preferred Qualifications:
- BSN degree
- Experience managing 2532 point caseloads per week
- Strong knowledge of Medicare and home health compliance standards
