An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patient homes within Upper Marlboro, MD. This role focuses on providing quality, patient-centered care through comprehensive assessments, individualized care plans, and collaboration with interdisciplinary healthcare teams. Candidates should be well-organized, self-motivated, and capable of working independently while upholding clinical excellence and effective patient communication.
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop care plans tailored to patient needs.
- Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress and promptly communicate any changes to the care team and physicians.
- Educate patients and families on treatment plans, medications, disease processes, and preventive care.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care.
- Participate in case conferences, staff meetings, and continuing education activities.
- Ensure adherence to regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing within the state.
- Minimum of 1 year recent home health experience with field experience preferred.
- Proficiency in HCHB and OASIS assessment tools.
- Strong clinical assessment and critical thinking abilities.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, organizational, and time management skills.
- Ability to work independently and manage a flexible field-based schedule. Preferred:
- BSN degree.
- Experience managing a caseload of 2532 points weekly.
- Knowledge of Medicare and home health compliance standards.
Schedule & Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM 5:00 PM.
- Caseload of 35 patient visits per day (2532 points weekly).
- Field-based position requiring daily travel within an assigned territory (mileage reimbursement provided).
