Bring your clinical expertise to patient-centered care in the home health setting as a skilled Registered Nurse. Delivering compassionate nursing support, youll have the opportunity to make a tangible difference in the lives of patients while enjoying the autonomy of a flexible, field-based schedule.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Daily travel within the assigned territory (mileage reimbursement provided)
- Caseload: 2532 points weekly (approximately 35 patient visits per day)
Key Responsibilities:
- Perform comprehensive OASIS assessments and create personalized care plans.
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient status, identify and communicate any changes to physicians and the care team promptly.
- Provide education regarding disease management, medications, and preventive health to patients and their families.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary colleagues including therapists, social workers, and home health aides to support coordinated care.
- Participate in case conferences, team meetings, and required training.
Desired Qualifications:
- Active RN license in good standing
- At least one year of recent Home Health RN experience (field expertise strongly preferred)
- Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS documentation
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent clinical assessment, time management, and communication skills
- BSN, experience with weekly point management, and strong Medicare/compliance knowledge preferred
