Deliver compassionate, high-quality nursing care directly to patients in the comfort of their homes by joining a dynamic, patient-focused team. This field-based Home Health Registered Nurse position offers the opportunity to make a significant difference in patients lives, driving positive health outcomes and fostering independence through clinical excellence and education.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive assessments utilizing OASIS and develop tailored care plans for each patient under your care.
• Deliver skilled nursing interventions, including medication administration, wound care, IV therapy, and chronic disease management.
• Monitor and evaluate patient conditions, communicating changes promptly with physicians and the interdisciplinary team to ensure seamless coordination.
• Provide thorough education to patients and families regarding disease processes, medications, and preventive strategies.
• Maintain meticulous and timely electronic documentation in Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, medical social workers, and aides.
• Participate in team meetings, case conferences, and professional development opportunities.
• Ensure adherence to regulatory, documentation, and agency standards.

Skills and Experience:

• Active RN license in good standing.
• Minimum 1 year of recent field-based Home Health RN experience.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
• Strong clinical assessment, organization, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to independently manage a field-based schedule.
• Preferred: BSN; prior experience managing a 2532 point weekly caseload; familiarity with Medicare home health compliance.
