Become a vital part of patient-centered care as a Home Health Registered Nurse, delivering high-quality nursing services to individuals in their own homes. This is a full-time contract position spanning 13 weeks, ideal for professionals driven by independence and a passion for making a difference in the community.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health experience (field work preferred)
• Hands-on proficiency with Home Care Home Base (HCHB) documentation
• OASIS assessment experience is essential
• Solid clinical assessment and critical thinking abilities
• Dependable vehicle, drivers license, and valid auto insurance
• Self-motivation and the ability to manage a flexible field-based schedule
• Exceptional communication, time management, and organizational skills

Preferred:

• BSN degree
• Experience with 2532 points per week caseload
• Understanding of Medicare/home health compliance standards

Responsibilities:

• Complete thorough OASIS assessments and develop patient-specific care plans
• Administer skilled nursing care such as wound management, medication administration, IV therapy, and chronic condition monitoring
• Track patient progress, report changes, and collaborate effectively with physicians and care teams
• Provide patient and family education on treatment and ongoing care
• Maintain precise, timely documentation in HCHB
• Work closely with therapists, social workers, and aides for holistic support
• Participate in case conferences, staff meetings, and relevant training activities
• Uphold all regulatory and agency standards in documentation and care delivery

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
