Job Description
Experienced and compassionate registered nurses with a background in home health are invited to provide patient-centered care in a field-based role serving individuals in their homes. This opportunity emphasizes autonomy, clinical excellence, and strong communication while supporting patients optimal health and well-being.
Schedule & Contract Details:
- Contract type: Full-time, 13-week assignment
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Setting: Field-based (providing care in patients' homes)
- Daily travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing services including wound care, medication management, IV therapy, and chronic disease support
- Monitor patient progress and quickly respond to changes in condition, coordinating with other team members and physicians as needed
- Educate patients and family members regarding treatment plans, medications, and disease management
- Accurately document all visits and interventions using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary professionals to ensure seamless, high-quality care
- Participate in case conferences, staff meetings, and ongoing professional development
- Comply with regulatory and agency standards for care delivery and documentation
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum 1 year of recent home health experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments required
- Excellent clinical assessment, critical thinking, time management, and organizational skills
- Reliable personal transportation, valid drivers license, and auto insurance
- BSN and experience managing a full point caseload preferred
- Familiarity with Medicare and home health compliance is an asset
