A rewarding opportunity awaits an experienced and compassionate Registered Nurse specializing in home health care. This contract position offers the chance to provide skilled, patient-centered nursing services directly in individuals' homes. The ideal candidate is self-motivated, detail-oriented, and thrives when working independentlydelivering high quality care and maintaining excellent communication.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract
• 2532 points per week caseload (typically 35 patient visits daily)
• Daily travel within the assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver a range of skilled nursing tasks, including wound care, medication administration, IV therapy, and chronic disease management.
• Frequently evaluate patient progress and communicate updates with the care team and physicians.
• Provide education to patients and families regarding conditions, treatments, and medications.
• Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate with other healthcare professionals to ensure coordinated, quality care.
• Participate in meetings, case conferences, and ongoing education as needed.
• Ensure compliance with regulatory and agency requirements.

Qualifications:

• Active RN license in good standing.
• At least one year of recent home health (field) experience.
• Proficient in Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to independently manage a field-based schedule.

Preferred:

• BSN degree.
• Previous experience handling a 2532 point weekly caseload.
• Familiarity with Medicare and home health compliance standards.
