Job Description

Elevate patient care where it matters mostat home. An immediate opportunity is available for an experienced and driven Registered Nurse specializing in home health, providing skilled services to patients within the comfort of their residences. This 13-week full-time contract requires dedication, compassion, and the ability to balance autonomy with outstanding clinical standards.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (35 patient visits daily)
  • Field-based: Daily travel to patient homes within your assigned area

Key Responsibilities:

  • Complete comprehensive OASIS assessments to tailor individualized care plans.
  • Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic illness management.
  • Track and evaluate patient progress, swiftly escalate changes, and collaborate with physicians and the care team.
  • Educate patients and their families on treatment regimens, medication, preventive care, and health management.
  • Ensure thorough and timely documentation using Home Care Home Base (HCHB).
  • Work closely with interdisciplinary professionalsphysical therapists, occupational therapists, social workers, and home health aides.
  • Participate in ongoing professional development, case conferences, and staff meetings.
  • Uphold strict compliance with regulatory and organizational standards in all aspects of patient care and documentation.

Qualifications:

  • Active RN license (state of practice) in good standing.
  • At least 1 year of recent home health nursing experience (field preferred).
  • Proficient in Home Care Home Base (HCHB) and OASIS.
  • Reliable transportation, valid drivers license, and up-to-date auto insurance.
  • Demonstrated independence, flexibility, and exceptional clinical, communication, and organizational skills.
  • BSN and experience with Medicare and managing higher caseloads are preferred.
