A skilled and compassionate Home Health Registered Nurse (RN) is sought for a rewarding, full-time contract role providing patient-centered care in a home environment. This 13-week assignment offers a dynamic field-based setting, focusing on delivering high-quality nursing services and promoting optimal health outcomes for patients.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Expect 2532 points per week, typically 35 patient visits daily

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease support
• Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians
• Provide thorough education to patients and caregivers regarding diagnoses, medications, and care strategies
• Ensure accurate, timely documentation using Home Care Home Base (HCHB)
• Coordinate with interdisciplinary teams, including physical and occupational therapists, medical social workers, and home health aides
• Participate in case conferences, meetings, and continuing education as appropriate
• Uphold all clinical, regulatory, and agency documentation standards

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent Home Health field experience required
• Proficiency in Home Care Home Base (HCHB) and prior OASIS experience required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, critical thinking, and independent decision-making skills
• Excellent communication, time management, and organizational abilities
• BSN preferred, and prior experience handling a caseload of 2532 points weekly is an advantage
• Knowledge of Medicare and home health compliance standards is a plus

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
