An opportunity is available for an experienced Home Health Registered Nurse to deliver skilled in-home nursing care to a diverse patient population. This contract role emphasizes patient-centered, high-quality care in a supportive, field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Manage a caseload of 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop customized care plans.
• Provide direct skilled nursingincluding wound care, medication management, IV therapy, and chronic disease support.
• Regularly monitor patient progress, identify changes in status, and coordinate with physicians and the broader healthcare team.
• Educate patients and families about disease management, treatment adherence, and preventive care strategies.
• Document all care accurately and promptly using Home Care Home Base (HCHB).
• Collaborate effectively with interdisciplinary team members such as therapists and social workers.
• Participate in meetings, case conferences, and ongoing education.
• Maintain compliance with all regulatory guidelines and best practices.

Qualifications:

• Active RN license in good standing.
• At least 1 year of recent field-based Home Health RN experience.
• Proficient with OASIS and Home Care Home Base (HCHB) electronic documentation.
• Strong clinical assessment, communication, and organizational skills.
• Reliable personal transportation, valid driver's license, and auto insurance.
• Ability to work independently and manage a dynamic home visit schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing a 2532 point weekly caseload.
• Familiarity with Medicare and home health compliance requirements.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
