Key Responsibilities:

• Perform comprehensive assessments of patients in their home environments
• Develop and implement individualized care plans in collaboration with patients, families, and the broader healthcare team
• Provide skilled nursing care, including medication management, wound care, and disease education
• Monitor patient progress, update care plans as needed, and communicate changes to physicians
• Educate patients and families on treatment, self-management, and preventative care strategies
• Document care provided in accordance with regulatory and accreditation requirements
• Comply with infection prevention protocols and ensure patient safety at all times

Qualifications:

• Current and unrestricted RN license
• Proven Home Health RN experience required
• Strong clinical assessment and critical thinking abilities
• Excellent interpersonal and communication skills, both written and verbal
• Ability to work independently and manage multiple cases
• Proficiency in electronic medical records/documentation systems
• Reliable transportation and valid drivers license
