Deliver expert nursing care and compassion in a dynamic home health setting. This position offers the opportunity to make a direct impact on patient well-being by providing tailored care plans, education, and support to individuals in the comfort of their own homes.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medication, wound care, and other nursing treatments as prescribed
- Collaborate with multidisciplinary team members, including physicians and therapists, to ensure coordinated care
- Educate patients and families on disease processes, medications, and self-care techniques
- Monitor changes in patients conditions and communicate findings promptly to providers
- Maintain accurate and thorough clinical documentation in compliance with home health regulations
- Support patients with chronic illnesses, recovery post-surgery, and palliative needs, ensuring their comfort and safety
Qualifications & Experience:
- Current, unrestricted RN license
- Previous experience in home health nursing strongly preferred
- Proficiency with OASIS charting and home health documentation standards
- Strong clinical assessment, critical thinking, and patient education skills
- Ability to work independently, manage time effectively, and adapt to diverse home care environments
- Reliable transportation and valid drivers license required
