Schedule & Assignment

• Monday Friday, 8:00 AM 5:00 PM
• 13-week contract, full-time
• Expectation of 2532 points per week, typically 35 patient visits per day
• Field-based, with daily travel to patient homes within an assigned territory

Key Responsibilities

• Conduct comprehensive OASIS assessments; develop and manage individualized care plans
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Closely monitor patient progress, recognize changes in condition, and communicate with the care team and physicians promptly
• Educate patients and their families on care plans, medications, and health management strategies
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams, including therapists, social workers, and home health aides, for coordinated care
• Participate in case conferences, staff meetings, and ongoing training
• Adhere to all regulatory and documentation standards

Qualifications

• Active RN license in good standing within the state
• Minimum of 1 year recent home health (field) experience
• Proficiency in Home Care Home Base (HCHB) and OASIS assessment
• Reliable transportation, valid drivers license, and current auto insurance
• Strong clinical and critical thinking skills
• Effective communicator able to work both autonomously and collaboratively

Desirable Attributes

• BSN preferred
• Prior experience with caseloads equivalent to 2532 points per week
• Familiarity with Medicare and compliance standards
