Experienced and compassionate nursing professionals are needed to provide skilled care to patients in their homes. This contract assignment offers a chance to make a meaningful difference by delivering high-quality, patient-centered care in a home health setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Administer skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify health status changes, and communicate effectively with interdisciplinary care teams and physicians.
• Provide patient and family education regarding treatment plans, medications, and prevention strategies.
• Accurately document care in Home Care Home Base (HCHB) software.
• Coordinate with therapists, medical social workers, and home health aides for optimal care delivery.
• Attend case conferences, meetings, and continue professional development as needed.
• Maintain compliance with regulatory and agency documentation standards.

Required Qualifications:

• Active RN license (in good standing).
• Minimum one year of recent home health experience; field experience preferred.
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Effective clinical assessment, time management, and organizational abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.

Preferred Qualifications:

• BSN degree.
• Previous experience managing 2532 points per week.
• Familiarity with Medicare and home health compliance standards.
