Delivering expert and compassionate care in patients homes, youll thrive as an experienced Home Health Registered Nursemaking a genuine impact on lives while advancing your skills in clinical excellence and patient advocacy. As the primary clinician in a field-based setting, youll complete comprehensive OASIS assessments, develop tailored care plans, and provide direct skilled nursing focused on ongoing patient progress, education, and safety.

Schedule

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (avg. 35 visits/day)

Responsibilities

• Conduct OASIS assessments and manage individualized care plans for patients in their homes.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status and promptly communicate changes to physicians and the care team.
• Provide thorough education to patients and families regarding care plans, medication regimens, and preventive health measures.
• Ensure meticulous electronic documentation using Home Care Home Base (HCHB).
• Partner with physical therapists, occupational therapists, social workers, and home health aides for coordinated, multidisciplinary care.
• Participate actively in team meetings, case conferences, and ongoing educational opportunities to maintain top-tier care standards.

Required Qualifications

• Active RN license (state of practice, in good standing)
• At least 1 year of recent home health experience (field-based preferred)
• Proficiency with HCHB and OASIS required
• Valid drivers license, auto insurance, and dependable transportation
• Strong assessment, critical thinking, and organizational skills
• Ability to independently manage a flexible caseload

Preferred

• BSN
• Experience with 2532 point weekly caseload
• Familiarity with Medicare and home health regulations

$2,490 - $2,634/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
