Step into a vital role providing compassionate, high-quality nursing care as a Home Health Registered Nurse. This rewarding contract opportunity is ideal for an experienced RN with a passion for supporting patients in a home-based setting. Youll play a key part in ensuring continuity of care, collaborating across disciplines, and guiding patients and their families to better health outcomes.
Schedule & Assignment Details:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Field-based, visiting patient homes daily (mileage reimbursement provided)
- Caseload: 2532 points per week (typically 35 patient visits per day)
Key Responsibilities:
- Conduct OASIS assessments and craft individualized care plans
- Deliver skilled nursing care (wound care, medication administration, IV therapy, chronic disease management)
- Closely monitor patient progress, communicating changes to healthcare teams and physicians
- Counsel and educate patients and families on disease management and prevention
- Accurately document all visits and care activities using Home Care Home Base (HCHB)
- Coordinate with interdisciplinary team members: PT, OT, medical social work, home health aides
- Engage in case conferences, staff meetings, and ongoing training
- Comply with regulatory and agency policies
Requirements:
- Active RN license (state-specific, in good standing)
- Minimum of 1 year recent home health (field) RN experience
- Proficient in Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, current auto insurance
- Strong assessment, communication, and organizational skills
- Ability to work independently with flexible field scheduling
Preferred:
- BSN degree
- Experience managing 2532 points/week
- Familiarity with Medicare and home health compliance

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
