Job Description
Deliver compassionate, high-quality care as an experienced Home Health Registered Nurse (RN). In this contract role, youll provide individualized skilled nursing services to patients in their homes, working autonomously to ensure continuity of care and positive patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Complete comprehensive OASIS assessments and develop tailored care plans.
- Deliver skilled interventions: wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, recognize changes in condition, and communicate updates to care teams and physicians.
- Educate patients and family members on disease processes, treatments, and preventive health practices.
- Maintain timely, accurate documentation in Home Care Home Base (HCHB).
- Collaborate with interdisciplinary partners such as PTs, OTs, social workers, and home health aides.
- Attend case conferences, staff meetings, and continue clinical education when necessary.
- Consistently adhere to regulatory and agency standards for care delivery and documentation.
- Provide daily travel to patient homes in your assigned area (mileage reimbursement available).
Required Qualifications:
- Active RN license in the state of practice, in good standing.
- At least 1 year of recent home health (field-based) experience.
- Proficient with Home Care Home Base (HCHB) and OASIS assessment tool.
- Reliable transportation, valid drivers license, and auto insurance.
- Strong communication, clinical assessment, and organization skills.
- Able to work independently with a flexible schedule.
Preferred Experience:
- BSN degree.
- Successful prior management of a 2532 point weekly caseload.
- Knowledge of Medicare and home health compliance requirements.
