A skilled and compassionate Registered Nurse is needed for a full-time home health contract role, focused on delivering high-quality care to patients in their own homes. This 13-week contract offers a MondayFriday schedule from 8:00 AM to 5:00 PM, with daily travel required to patient residences. The ideal candidate thrives in an autonomous environment, takes initiative in patient care, and prioritizes clear communication with patients, families, and the wider interdisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Daily field-based travel; mileage reimbursement provided
Required Qualifications:
- Active RN license in the state of practice, in good standing
- At least 1 year of recent field-based home health RN experience
- Proficiency using Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience
- Reliable vehicle, valid drivers license, and current auto insurance
- Superior clinical assessment, communication, and time management abilities
- Ability to work independently and manage a flexible caseload
Preferred Qualifications:
- BSN
- Experience managing 2532 points per week
- Familiarity with Medicare and home health compliance standards
Core Responsibilities:
- Conduct OASIS assessments and develop individualized patient care plans
- Deliver skilled nursing care: wound care, IV therapy, medication administration, and chronic disease management
- Track patient progress and communicate status changes to care teams and physicians
- Educate patients and families on health conditions, medications, and care plans
- Maintain thorough documentation in HCHB and collaborate with other home health professionals
- Participate in ongoing training, meetings, and case conferences
