An opportunity is available for a skilled and compassionate Home Health RN to deliver quality care to patients in their homes. This role is ideal for someone who thrives independently, demonstrates excellent clinical judgment, and enjoys building meaningful patient relationships.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily field-based work, typically 35 patient visits per day (2532 points/week)
Key Responsibilities:
- Conduct thorough OASIS assessments and craft individualized, goal-oriented care plans
- Provide skilled nursing care such as wound treatment, IV therapy, medication administration, and chronic disease management
- Carefully monitor clinical progress, communicating with physicians and interdisciplinary teams regarding patient status and changes
- Educate patients and families about disease management, medication adherence, and preventive strategies
- Maintain timely and comprehensive documentation using Home Care Home Base (HCHB)
- Collaborate effectively with therapists, social workers, and aides for cohesive patient-centered care
- Participate in regular case conferences, staff meetings, and learning opportunities
- Uphold regulatory, documentation, and agency standards with attention to detail
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum of 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment
- Strong clinical assessment, organization, and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a field-based schedule efficiently
- Excellent communication and time management skills
Preferred:
- BSN degree
- Previous experience managing 2532 points per week
- Familiarity with Medicare and home health compliance
