Step into a vital nursing role providing one-on-one, patient-centered care in the home setting. We are looking for an experienced and compassionate Registered Nurse skilled in home health to support patients as they recover and manage chronic conditions outside of the hospital. This opportunity offers a 13-week contract, allowing you to make a tangible impact while working autonomously with a supportive interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Role Highlights:
- Full-time, field-based (patient homes)
- Caseload: 2532 points per week (averaging 35 patient visits daily)
- Electronic charting utilizing Home Care Home Base (HCHB)
- Assessments performed using OASIS
- Daily travel within a manageable territory (mileage reimbursement included)
Key Responsibilities:
- Complete comprehensive OASIS assessments and create tailored care plans
- Deliver direct skilled nursing care: wound care, IV therapy, medication administration, chronic disease management
- Monitor patient progress, identify status changes, and coordinate with providers
- Educate patients and families on care, treatments, and preventive strategies
- Maintain accurate, timely clinical documentation
- Collaborate closely with therapists, social workers, and aides to ensure seamless support
- Attend team conferences, meetings, and ongoing training as needed
- Uphold regulatory, clinical, and agency documentation standards
Requirements:
- Active state RN license (in good standing)
- Minimum 1 year recent home health RN experience (field experience preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Excellent patient assessment, communications, and organizational abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work independently and adapt in a field-based environment
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
