An excellent opportunity is available for a skilled and compassionate Registered Nurse with experience in home health care. This contract role is ideal for an RN who is motivated, organized, and comfortable working independently to deliver exceptional care in a field-based setting. You will play a crucial part in ensuring quality, individualized nursing for patients in their homes, focusing on patient-centered outcomes and strong communication.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized plans of care.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify and respond to changes in condition, and communicate effectively with care teams and physicians.
• Educate patients and families on treatment plans, medications, and preventive health.
• Document all care and communications accurately and efficiently using Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary team including physical and occupational therapists, social workers, and home health aides.
• Attend case conferences, staff meetings, and ongoing professional development sessions as required.
• Comply with regulatory and agency standards related to clinical documentation and patient care.

Required Qualifications:

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year recent home health field experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments.
• Strong assessment abilities, critical thinking, and organizational skills.
• Reliable transportation, valid driver's license, and current auto insurance.
• Ability to manage a flexible, field-based schedule and caseload of 2532 points weekly.

Preferred Qualifications:

• BSN degree.
• Experience with Medicare and home health compliance.
• Prior management of a similar caseload.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
