An experienced Home Health Registered Nurse (RN) is needed to provide compassionate, skilled nursing care to patients in their homes. This role demands a self-motivated professional who can independently deliver high-quality clinical care while communicating effectively with patients and healthcare teams.
In this position, the RN conducts thorough OASIS assessments, develops individualized care plans, administers treatments such as wound care and IV therapy, and monitors patient progress. Coordination with interdisciplinary team members ensures comprehensive, patient-centered care. Accurate and timely documentation using Home Care Home Base (HCHB) is essential, as is adherence to all regulatory standards.

This opportunity is based in Stone Mountain, GA, a vibrant community known for its rich history, outdoor activities, and welcoming neighborhoods. Residents enjoy access to parks, cultural attractions, and proximity to metropolitan amenities, creating a balanced work-life environment.
Schedule/Hours:
- Full-time, 13-week contract
- Monday through Friday, 8:00 AM 5:00 PM
- Approximately 35 patient visits daily, managing 2532 points per week
- Field-based travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Perform comprehensive OASIS assessments and care planning
- Provide skilled nursing including medication administration, wound care, IV therapy, and chronic disease management
- Monitor patient progress and communicate changes to physicians and care team
- Educate patients and families on medical conditions and care plans
- Maintain accurate documentation in HCHB
- Collaborate with therapists, social workers, and home health aides
- Participate in case conferences and continuing education
- Follow all agency and regulatory documentation and care standards
Qualifications / Desired Experience:
- Active RN license in good standing within the state of practice
- Minimum 1 year recent home health experience; field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with flexible scheduling
- BSN preferred
- Experience managing 2532 points weekly and Medicare/home health compliance knowledge advantageous
