Key Responsibilities:

• Administer direct nursing care, including assessments, medication administration, wound care, and monitoring of patient progress in the home environment.

• Educate patients and family members on disease management, medication regimens, and preventative health measures.
• Accurately document patient assessments, interventions, and outcomes in a timely manner.
• Conduct regular visits and respond promptly to changes in patient status, emergencies, or unique care needs.
• Maintain compliance with all healthcare regulations and standards while upholding the highest ethical and professional standards.

Required Qualifications:

• Current and unencumbered RN license.
• Proven experience in home health nursing or community-based care is strongly preferred.
• Solid knowledge of home care protocols and best practices, with the ability to operate autonomously.
• Strong interpersonal communication skills, empathy, and adaptability to a variety of home environments.
• Proficiency in EMR/electronic documentation systems is advantageous.
• Must possess reliable transportation and a valid drivers license.
