This position focuses on delivering compassionate and specialized care to patients in the comfort of their homes, supporting their wellness and independence in a critical and rewarding setting.
Schedule/Hours:
- Flexible scheduling options available
- Day shifts with some on-call rotation as needed
Key Qualifications:
- Active RN license in the state of Georgia
- Minimum one year of recent experience in home health nursing
- Strong assessment, case management, and communication abilities
- Proficient with electronic medical records and mobile documentation
- Exceptional organizational and critical thinking skills
- Valid drivers license and reliable transportation
Primary Responsibilities:
- Conduct comprehensive initial assessments and develop individualized care plans for homebound patients
- Administer medications, wound care, and other skilled nursing procedures as prescribed
- Coordinate multidisciplinary care in collaboration with physicians and therapists
- Educate patients and families on treatment plans, preventive care, and disease management
- Monitor, record, and report symptoms or changes in patient conditions
- Ensure the safety and comfort of patients in a home environment
