Experienced Registered Nurses are invited to deliver high-quality, patient-centered care as part of a dedicated home health care team. This full-time contract position spans 13 weeks, offering the opportunity to work independently and make a meaningful impact in patients lives by providing skilled nursing services directly in their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and implement individualized care plans to meet patient needs.
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor and evaluate patient progress, promptly reporting changes to interdisciplinary teams and physicians.
• Educate patients and families on illness management, self-care, and preventive healthcare strategies.
• Maintain accurate, real-time documentation using Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, social workers, and aides to provide well-coordinated care.
• Actively participate in staff meetings, case conferences, and professional development.

Required Qualifications:

• Active, unencumbered RN license in the state of practice.
• Minimum of 1 year recent field-based home health RN experience.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS documentation.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong assessment, critical thinking, and communication skills.
• Ability to work autonomously while managing a flexible, field-based schedule.

Preferred:

• Bachelor of Science in Nursing (BSN) degree.
• Prior experience managing a weekly caseload of 2532 points.
• Understanding of Medicare and home health compliance standards.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
