Bring your expertise to a dynamic Home Health RN role, delivering high-quality, patient-centered care in the comfort of patients homes. Enjoy autonomy and variety as you conduct skilled nursing visits, build relationships with patients and families, and coordinate care with an interdisciplinary team. This is a full-time, 13-week contract assignment with a focus on excellence, professional growth, and meaningful patient impact.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• 2532 points per week (roughly 35 patient visits per day)
• Field-based, with daily travel to patient homes

Key Responsibilities:

• Use expert clinical judgment to complete OASIS assessments and develop individualized plan-of-care for each patient.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and management of chronic conditions.
• Monitor patient progress, communicate any changes to the interdisciplinary team, and ensure all care complies with physician directives.
• Educate patients and caregivers about disease management, treatment, and preventive healthcare.
• Accurately document all care activities in Home Care Home Base (HCHB) in a timely manner.
• Actively participate in case conferences and ongoing professional development.

Desired Qualifications:

• Active RN license (good standing in your state of practice)
• At least 1 year of recent home health experience, ideally field-based
• Proficiency in HCHB and OASIS required
• Exceptional assessment, organizational, and communication skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently and adapt to a flexible schedule
• BSN and prior experience managing 2532 points per week preferred
• Familiarity with Medicare and home health compliance is a plus

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
