Seeking a skilled and compassionate Registered Nurse to deliver outstanding patient-centered care in a home health setting. This contract-based opportunity is ideal for an RN who thrives on autonomy, values continuity in patient relationships, and excels at coordinating complex care in patients homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (approximately 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct detailed OASIS assessments and develop personalized care plans.
- Perform skilled nursing duties, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor, evaluate, and communicate patient progress to the interdisciplinary care team.
- Educate patients and families on health maintenance, medications, and post-acute care plans.
- Accurately complete electronic documentation using Home Care Home Base (HCHB).
- Coordinate care with physical therapists, social workers, home health aides, and other professionals.
- Maintain regulatory and agency standards for care delivery and documentation.
- Participate in case conferences, staff meetings, and ongoing education as necessary.
Required Qualifications:
- Current RN license (state of practice, in good standing)
- At least 1 year of recent home health experience (field experience strongly preferred)
- Proficiency in Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment abilities and decision-making skills
- Reliable transportation, valid drivers license, and active auto insurance
- Self-directed, with excellent communication and organizational abilities
Preferred Background:
- BSN degree
- Experience managing 2532 points per week
- Understanding of Medicare and home health compliance
