Experienced and compassionate Registered Nurses are needed for a dynamic home health opportunity, delivering skilled care directly to patients in their homes. In this field-based role, youll manage a caseload with a strong focus on quality, patient-centered outcomes and enjoy the autonomy of practicing independently within a supportive healthcare framework.
Schedule & Details:
- Contract position, Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
- Typical caseload: 2532 points per week (averaging 35 patient visits daily)
- Field-based: daily travel with mileage reimbursement
- Documentation: Home Care Home Base (HCHB) and OASIS assessment tool
Your Responsibilities:
- Complete comprehensive OASIS assessments and tailor individualized care plans
- Deliver skilled nursing: wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient status, communicate changes to the healthcare team, and collaborate with other disciplines (PT, OT, MSW, and aides)
- Educate patients and families regarding treatment plans, medications, and disease management
- Maintain accurate and timely electronic clinical documentation in HCHB
- Participate in case conferences, staff meetings, and ongoing education as necessary
- Adhere to regulatory and agency standards for care delivery
Qualifications:
- Active RN license in good standing (state of practice)
- At least 1 year of recent home health experience, field preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Strong assessment, clinical, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN preferred; experience with 2532 point caseload and Medicare knowledge are pluses
