Seeking a skilled and compassionate Registered Nurse to deliver high-quality care in a home health setting. This contract opportunity involves visiting patients in their homes, coordinating care, and supporting their health goals independently while partnering with a multidisciplinary team.

Schedule:

• MondayFriday, 8:00 AM5:00 PM
• 13-week contract
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Daily travel within assigned territory (mileage reimbursement available)

Key Responsibilities:

• Conduct comprehensive patient assessments using OASIS and create individualized care plans.
• Deliver skilled nursing such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patients' progress, communicate changes to physicians and care teams.
• Educate patients and families regarding treatments, medications, and healthy living practices.
• Document all care in the Home Care Home Base (HCHB) electronic system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated patient care.
• Participate in case conferences, meetings, and ongoing education.
• Maintain compliance with regulatory and organizational standards.

Qualifications:

• Active RN license, in good standing, in the state of practice.
• At least 1 year of recent field-based Home Health experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong assessment and clinical judgment abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent communication, organizational, and independent time management skills.
• Preferred: BSN, prior experience managing 2532 points per week, and familiarity with Medicare and home health compliance.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
